After some testing, it appears that Quests Extended version 3.3.0 is the ideal version to pair with for the time being. If you have an issue where some of the quest conditions are not counting, consider downgrading your Quests Extended version to 3.3.0.

Oh, sorry, I should probably add that this is more or less an *alternative* to any other medical system and therefore incompatible 😅 I'm happy that you were able to arrive at a solution, and if it works best for you, you should keep it that way. However, the intended configuration is to just choose one or the other. Even though I believe the original mod was designed for both, I do not personally use Realism's medical changes, as much as I want the hazard zones- they are unfortunately incompatible with Fika.

Oh yes. At most, it might change some of the resource values of existing medical items, so you may see oddities such as 1/2 use bandages, or 3/2 use caloks.
All of the custom items are brand new IDs, so they won't harm anything on an existing profile. If you obtain some of these items and then remove the mod without deleting the custom items, that's where you could encounter issues.

Hi there Mr Fountain. I'm a big fan of your mod for its *systems*, but as far as balance goes, real life is cruel in the realm of balance. I aimed to make a more arcadey spread so that finding or buying.. for example, an endgame helmet such as a Rys-T would feel rewarding. I wasn't happy that helmets are practically vanity items against even basic rifle rounds. And while I understand that that is how they are in reality, for a game I drummed up my own vision.
